• Your electronic signature has the same legal force and effect as a handwritten signature under the federal Electronic Signatures in Global and National Commerce Act (E-SIGN Act) and applicable state law

• Documents may be forwarded to the Health Insurance Marketplace, insurance carriers, or government agencies as required for verification and enrollment
• Submitting forged, altered, or fraudulent documents is a violation of federal and state law and may result in criminal prosecution
• We retain copies of submitted documents as required for compliance and record-keeping purposes
• Attestation forms, including Medicare/Medicaid attestations and income declarations, are legal statements made under penalty of perjury

7. Payment and Financial Information

When you provide banking or payment information through our ACH update forms, you authorize United Liberty Insurance Agency to process the requested transactions and share necessary payment information with insurance carriers for premium payment processing. We do not store complete banking credentials on our servers. You are responsible for ensuring the accuracy of all financial information provided and for maintaining sufficient funds for scheduled premium payments.

8. Intellectual Property

All content on this Site, including but not limited to text, graphics, logos, images, tools, calculators, form designs, and software, is the property of United Liberty Insurance Agency or its licensors and is protected by United States copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, create derivative works from, publicly display, or exploit any content from this Site without our prior written consent.
